You selected the issue: Headset not detected (error 108 / 125 / 126 / 211)
Troubleshooting:
Reboot your headset
  1. Right-click on the headset icon in SteamVR
  2. Select Reboot Vive Headset
  3. Wait for the reboot process to complete
Power cycle your Link Box
  1. Quit SteamVR
  2. Unplug the Power and USB cords from the PC side of the Link Box (not the orange side)
  3. Wait a few seconds, then plug the Power and USB cords back into the Link Box
  4. If you see more drivers install, wait for this process to finish
  5. Launch SteamVR
Enable Direct Mode
Launch SteamVR and go to SteamVR > Settings > Developer tab > Enable Direct Mode

Direct Mode ensures that your HMD is not recognized as a monitor. If you close SteamVR and the HMD display shows your desktop, this means that Direct Mode is not enabled. When Direct Mode is running properly, you will only see VR content in the HMD.

If you have trouble enabling Direct Mode, make sure that you've installed the most recent drivers for your GPU.
Connect to a different USB port
Plug your Link Box into a different USB port. If you're using a USB 2.0 port, switch to a USB 3.0 port (blue interior). If you're using a USB 3.0 port, switch to a USB 2.0 port.

Try a USB port that's already in use by another device and working.

If you've tried all of the available troubleshooting and continue to have issues, your USB chipset may be at fault. Our testing has shown that the Inateck 2-Port USB3.0 PCI-Express Card (Serial: KTU3FR-2O2I) works reliably with the HTC Vive and can overcome these kinds of USB issues.
Restart your computer
Hit the X on SteamVR to quit, quit the Steam client, then restart your computer.
